Why do we talk about CBD oil as a pain reliever?

The cannabidiol or CBD is a natural anti-inflammatory that acts on the receptors of the endocannabinoid system, a system responsible for maintaining the body's homeostasis. CBD works to rebalance this system when a stimulus, such as pain, causes an imbalance. In particular, it interacts with CB2 receptors, which are mainly found in immune system cells and in some areas of the central nervous system, promoting the modulation of the inflammatory response.

Furthermore, the CBD modulates the GABAergic system, the brain's primary inhibitory system, helping to reduce the perception of pain. It can also interact with TRPV1 receptors, known as vanilloid receptors, which are involved in pain perception and body temperature regulation. The activation of these receptors by the CBD can promote the desensitization of pain pathways, naturally reducing the painful sensation.

CBD drops for pain: what does science say?

The CBD, which is a non-toxic and non-psychoactive compound, has been the subject of numerous studies that have shown its usefulness in management of different forms of pain, predominantly refractory to opioid treatment.  

CBD and inflammatory pain

CBD has been found useful to support the body in case of inflammatory pain. In fact, in many inflammatory conditions, such as arthritis, CBD acts as a natural anti-inflammatory and prevents its worsening. In a study conducted by theInsubria University of Varese has been highlighted that cannabis oil can help inhibit the production of cytokines, which are inflammatory molecules. Cannabidiol also acts on certain chemical processes of anandamide, an endocannabinoid linked to pain perception. CBD is often used when inflammation is at the root of the problem. For example, it has been found effective for the vulvodynia and for the cervical pain.

CBD and chronic pain

Most studies on CBD and chronic pain have indicated a pain reduction ranging from 43 to 66%, a rather promising result especially for such a troublesome and complex condition. One study published on Journal of Pain on cannabis and chronic pain  has focused on demonstrating the safety of CBD, which is in every respect a safe and natural molecule.

In an interesting survey conducted in 2021 on 878 people suffering from fibromyalgia, a syndrome characterized by widespread chronic pain, CBD proved to be an effective natural alternative for 72% of the participants, who used it instead of traditional medications for chronic pain, such as NSAIDs, opioids, and benzodiazepines. Those who took part in the survey reported better pain and sleep management and a reduction or complete discontinuation of medications.

CBD and neuropathic pain

Some studies they have demonstrated the effectiveness of cannabidiol on neuropathic pain. In particular, it has proven promising against the peripheral neuropathic pain caused by chemotherapeutics and in the regulation of adenosine receptors, involved in the transmission and chronicization of pain.

The effects of CBD are not limited to chronic neuropathic and inflammatory pain, but have also been observed in many other clinical conditions, such as multiple sclerosis, epilepsy, insomnia, schizophrenia, and many other diseases resistant to drug therapy. According to a scientific review on CBD and pain, cannabidiol is an effective pain reliever capable of improving not only the painful condition but also sleep and quality of life overall.

The peculiarity of CBD, in fact, is that its action is not limited to modulating the inflammatory response. This cannabinoid can interact with serotonin receptors, a neurotransmitter involved in regulating mood and anxiety. This interaction allows CBD to exert an anxiolytic and relaxing effect, which can be particularly valuable for those suffering from chronic pain.

CBD for joint pain: a study on arthritis

According to one study on the effectiveness of cannabidiol for joint pain, CBD oil or hemp oil with CBD can help reduce joint pain by up to 44%.

The study used an anonymous questionnaire to assess the effectiveness of cannabidiol on different types of arthritis: osteoarthritis, rheumatoid arthritis, and autoimmune arthritis  The test was conducted on a sample of 428 volunteers.

83% of people who used CBD reported an overall improvement in pain, 66% also associated cannabidiol with an improvement in physical function and sleep quality. Compared to different types of arthritis, the greatest benefit was reported by participants with osteoarthritis issues. The same study highlighted how the use of CBD helps reduce the use of anti-inflammatory drugs. 259 participants reported a decrease or even cessation of the use of medications such as anti-inflammatories, paracetamol, and opioids.

Another search on CBD and arthritis rheumatoid highlights an anti-arthritic activity by cannabidiol, which could improve arthritis by targeting synovial fibroblasts under inflammatory conditions.

Is hemp oil an anti-inflammatory?

Hemp oil is known for its anti-inflammatory properties. This is largely due to its composition rich in essential fatty acids, including alpha-linolenic acid (omega-3) and linoleic acid (omega-6), which play a crucial role in reducing systemic inflammation.

Unlike many food sources that contain excess omega-6, hemp oil has a balanced ratio of omega-3 to omega-6, usually 1:3. This balance is essential for maintaining a healthy inflammatory response. Additionally, omega-3 and omega-6 are precursors to prostaglandins, hormone-like substances that modulate the inflammatory process. Omega-3s in particular stimulate the production of anti-inflammatory prostaglandins, while omega-6s contribute to cell regeneration without triggering inflammation.

In addition to omega-3 and omega-6, hemp oil also contains a range of natural antioxidants, including vitamin E. Antioxidants help combat oxidative stress, which is one of the main causes of chronic inflammation in the body.

Although it has anti-inflammatory properties, hemp oil is not a painkiller, but it is an excellent carrier oil that effectively supports the anti-inflammatory and pain-relieving action of cannabidiol.

CBD and pain therapy

Durante l'interview, Calvanese told us that there are numerous studies on the subject. In particular, studies conducted by doctors Massimo Nabissi and Marco Falasca within a project that started at Shanghai University and continued in Italy. 

These are studies that illustrate therapeutic innovations for certain types of cancer, which also include the administration of cannabis sativa and CBD. Researchers have discovered that both pancreatic carcinoma and glioblastoma multiforme, two very aggressive forms of tumors, express a protein called CBR55. 

The cannabidiol has the ability to act directly on this protein, blocking it and triggering programmed cell death. Calvanese argues that for an oncology patient, undergoing a therapeutic plan based on cannabis can be advantageous and bring several benefits. In particular, CBD helps to significantly reduce pain and stimulate appetite, which often disappears after chemotherapy cycles and moral decline. 

CBD and Therapeutic Cannabis

Given its beneficial properties, cannabidiol is also present in therapeutic cannabis preparations, that is, cannabis for medicinal use, which can only be taken with a medical prescription for specific conditions. When referring to therapeutic cannabis, we mean “Cannabis FM-2,” which contains a THC percentage between 5% and 8% and a CBD percentage between 7.5% and 12%, and is produced in Italy in compliance with European regulations on medicines. The production process is strictly controlled and takes place at an authorized pharmaceutical facility, managed by the Military Institute of Florence and under the supervision of the AIFA (Italian Medicines Agency). The distribution of therapeutic cannabis is regulated by the State Cannabis Agency, which operates within the Ministry of Health. Furthermore, for several years, some Italian pharmacies have been producing galenic preparations with THC concentrations that can reach up to 19%.

What is pain?

The pain is a sensation experienced due to specific nerve fibers that carry pain impulses from the periphery to the brain, which in turn integrates and modifies the pain information. This system is known as the ascending pain pathway. There is also a descending pain pathway, which sends signals from the brain to the periphery carrying the message to turn off the pain. Various neurotransmitters such as glutamate or GABA are involved in these mechanisms. 

The pain can be of different types:

  • Sharp: the acute pain is sudden and has a short duration related to its cause, such as a surgical intervention, trauma, or localized inflammation.

  • Chronic: Chronic pain is characterized by the presence of persistent pain lasting more than six months, often even in the absence of an obvious cause. Unlike acute pain, which is a natural response of the body to an injury or inflammation and tends to disappear when the problem is resolved, chronic pain persists even after the triggering cause has disappeared. The painful sensation is often accompanied by other symptoms, such as chronic fatigue, muscle stiffness, and sleep disturbances, resulting in a significant impact on quality of life.

  • Neuropathic: a type of chronic pain that is particularly difficult to treat. Neuropathic pain is caused by nerve damage and can manifest as tingling, burning, or stabbing pain.
